肩甲上腕関節，肩峰下滑液包同時鏡視により腱板修復術を施行した関節面不全断裂の1例

Partial Articular-side Supraspinatus Tendon Avulsion Repaired by Simultaneous Glenohumeral Arthroscopy and Subacromial Bursoscopy. Case Report

　関節面不全断裂の1例に対して肩甲上腕関節および肩峰下滑液包を同時に鏡視しながら腱板修復する方法を試みた．まず肩甲上腕関節鏡視により断裂部とフットプリントのデブリドマンを施行し，ついで滑液包鏡視により肩峰下除圧を行った．この段階で同時鏡視とし，経腱板的にアンカーを用いて修復した．術後1年で疼痛は消失し，JOAスコアは術前の68点から98点へ改善した．経腱板的スーチャーアンカー刺入法では関節鏡視下にブラインドで操作が必要であるが，滑液包鏡視を同時に行うことにより容易に操作を行うことができた．

　We report a case repair of partial articular-side supraspinatus tendon avulsion (PASTA) guided by simultaneous arthroscopy and bursoscopy.　The frayed tendon fibers and footprint were abraded under intra-articular guidance by glenohumeral arthroscopy.　The arthroscope was then repositioned in the subacromial space, and acromioplasty was performed.　A second arthroscope was inserted into the glenohumeral joint and transtendon repair was performed under simultaneous vision by both arthroscopy and bursoscopy.　The JOA score had improved from 68 to 98 one year after surgery.　Our surgical technique enabled us to perform all of the procedures completely unblinded fashion.
