　DRV Locking Plateを用いたAO C3型橈骨遠位端骨折の一部にみられる術後橈骨短縮を防止するには，軟骨下骨部を格子状に固定することが有効であると考え，Kirschner鋼線(K-W)刺入ガイドを考案した．本手技を用いて手術を行った2例を経験したので報告する．2症例ともK-W2本とロッキングピンで格子状固定を行った．術後12カ月の最終観察時では2症例とも骨折部の整復位は保たれており，Cooney評価は優であった．本ガイドは容易かつ正確にしかも短時間でK-Wを刺入でき，有用なデバイスであると思われる．

　We developed a latticed guide through which Kirschner wire can be inserted into the subchondral bone of the radial distal fragment to prevent radial shortening after internal fixation of AO C3 fractures of the distal radius with a DRV Locking Plate.　We report 2 cases operated on using this new device.　The latticed fixation was achieved with two Kirschner wires and a locking pin in both cases.　As of the 12-month postoperative follow-up examination, the position of the fracture sites had been maintained, and the Cooney evaluation was excellent.　This new device seems to be useful because it permits rapid, easy, and accurate insertion of Kirschner wires.
