手関節尺側部痛が遺残した掌側ロッキングプレート施行橈骨遠位端骨折の3例

Ulnar Wrist Pain after Fixation of a Distal Radius Fracture with a Palmar Locking Plate. Report of Three Cases

　術中鏡視下に三角線維軟骨複合体（TFCC）を確認したが手関節尺側部痛を残した掌側ロッキングプレート施行橈骨遠位端骨折の3例について報告する．症例1は80歳の女性で，Colles骨折であった．TFCCは橈骨尺骨切痕付着部の一部に表層断裂を認めた．症例2は52歳の男性で，Colles骨折であった．TFCCは橈側付着部中央で部分断裂していた．トランポリン現象消失は判別困難であった．症例3は67歳の女性で，Colles骨折であった．TFCCは橈側付着部中央で断裂していた．尺側部痛の原因として，尺骨小窩でのTFCC 剝脱を見逃している可能性が考えられる．

　Three patients who underwent wrist arthroscopy had ulnar wrist pain after treatment of a distal radius fracture by palmar locking plate fixation. Case 1：The patient was an 80-year-old woman with a Colles fracture. After fixation with a palmar locking plate, wrist arthroscopy was performed to assess the triangular fibrocartilage complex (TFCC) injury, and a superficial radial tear was found. Case 2：The patient was a 52-year-old man with a Colles fracture. After fracture reduction and fixation with a palmar locking plate, the associated TFCC injury was assessed arthroscopically, and a partial radial avulsion tear was observed. Case 3：The patient was a 67-year-old woman with a Colles fracture. Wrist arthroscopy at the time of the operation revealed a partial TFCC injury on the radial side . We suspect that avulsion of the proximal ligamentous component of the TFCC, which can not be ovserved by wrist arthroscopy, may have contributed to the etiology of the ulnar wrist pain.
