　51歳の男性で，大腿内側広筋内に発生したガングリオンの症例を報告する．本邦での大腿四頭筋内ガングリオンの発生は稀である．本症例のガングリオンは内側広筋の抵抗減弱部である筋肉内に存在し，発生母地は膝蓋上囊であった．大腿四頭筋と大腿骨間の潤滑に関与している膝蓋上囊は常に外力が加わるところで，ガングリオンの発生を引き起こしたと考える．治療はガングリオンとその茎部を含めて一塊として摘出することである．

　We report the case of a ganglion in the vastus medialis muscle in a 51-year-old man. Reports of a ganglion in the quadriceps femoris muscle have been rare in Japan. The ganglion in our patient was in the low resistance compartment of the vastus medialis and originated in the suprapatellar pouch. The suprapatellar pouch participates in lubrication between the quadriceps femoris muscle and femur, and its constant exposure to external force may have been one of the cause of the ganglion. Treatment by en bloc excision of the ganglion and its stalk was successful.
