人工膝関節置換術の術後関節可動域―2種類のインプラントデザイン間の比較

Postoperative Range of Motion after Total Knee Arthroplasty: Comparison of Two Different Implant Designs

　2種類の異なるインプラントを用いた人工膝関節置換術（以下TKA）の術後屈曲角を検討した．Kinemax plusを用いた群（K群）とScorpioを用いた群（S群）の術前・後4週のX線像で屈曲角を計測した．屈曲角は後十字靱帯温存型，後十字靱帯切除型ともにS群で有意に大きかった．S群において深屈曲は46％で達成されていた．本研究は同一術者により同一手技で行われたもので，機種間の比較において利点を有する．

　We evaluated maximum postoperative flexion after TKA with 2 different implant designs, the Kinemax plus (K group) and the Scorpio (S group), by measuring the angle on radiographs before and 4 weeks after the operation. The S group had significantly greater flexion with both CR type(posterior cruciate retention design) and PS type(posterior stabiliser design) than the K group. Deep flexion was achieved in 46％ of the cases in the S group. The advantage of this study was that the designs were compared in patients operated on by the same surgeon by same procedure in which the parallel cut technique was used.
