　Headless tapered variable pitch compression screw（Acutrak^® screw）を使用した足関節固定術の1例を経験したので報告した．術後2年の観察期間で，疼痛が消失し，「正座できない」以外のADL制限もなく良好な成績が得られた．通常のスクリューの場合，関節固定部にネジ山がかからないように注意する必要があるが，本スクリューは全長にネジ山を持ち，骨片間に十分な圧迫固定力をかけることができる．Headlessのため骨内へ埋め込みが可能で，けい骨内側など軟部組織の少ない部位でも刺激が生じることがないなどの利点がある．

　A case of the ankle arthrodesis for post-traumatic arthritis in 60-year-old man using the headless tapered variable pitch compression screw (Acutrak^® screw) was reported. The union of the ankle joint was achieved after three months postoperatively. The follow-up was two years. Ankle pain was completely reduced and the excellent clinical result was gained. This Acutrak compression headless screw had tapering threads all over the length, then it was easily inserted and gained good compression force. For its headless form, the irritation at the screw head as observed in conventional screws did not occur.
