 Cloacal and bladder exstrophy are congenital disorders involving the genitourinary and intestinal tracts and the musculoskeletal system. The reconstruction of pelvic rings presents a considerable challenge. We conducted posterior innominate osteotomy in 3 cases of cloacal exstrophy and 1 of bladder exstrophy between 1982 and 1996. All patients had wide diastasis of pubic symphysis (average 6.5cm) preoperatively, which was corrected to 1.6cm postoperatively. At the latest follow-up, mean diastasis measured 1.8cm. All patients gained urinary continence and were reported a so-called toes-out gait.
