　総排泄腔外反症は，膀胱腸粘膜が外反・露出する重度の胎生初期の発生異常である．恥骨結合の離開を伴い，小児外科，泌尿器科的処置の際に締結を必要とする．この総排泄腔外反3例，膀胱外反症1例に対して，腸骨後方骨切り術を行い，平均11.9年（5.5～19.1年）の経過を観察した．術前の恥骨結合間距離は，平均65mm（36～88mm）であったものが，術直後は，16mm（6～28），最終観察時18mm（2～34）と離開せず，維持されていた．著明な外旋歩行を呈したものもなく，尿禁制も得られていた．

　Cloacal and bladder exstrophy are congenital disorders involving the genitourinary and intestinal tracts and the musculoskeletal system. The reconstruction of pelvic rings presents a considerable challenge. We conducted posterior innominate osteotomy in 3 cases of cloacal exstrophy and 1 of bladder exstrophy between 1982 and 1996. All patients had wide diastasis of pubic symphysis (average 6.5cm) preoperatively, which was corrected to 1.6cm postoperatively. At the latest follow-up, mean diastasis measured 1.8cm. All patients gained urinary continence and were reported a so-called toes-out gait.
