　抄録：リウマチ母指の指節間（IP）関節，中手指節間（MCP）関節に対して，Acutrak^® bone screwを用いて関節固定術を行い良好な成績を得ているので，手術法を紹介する．IP関節またはMCP関節を背側より露出し，cup and coneの形状になるように関節軟骨を切除する．軽度屈曲，やや回内位に保持してIP関節の場合には基節骨背側から，MCP関節の場合には第1中手骨背側からドリリングを行いAcutrak^® bone screwを末梢方向に刺入して固定する．IP関節の場合，末節骨先端から刺入すると固定角度は0°にならざるを得ないが，基節骨の背側から刺入することにより，固定角度を調節することが可能となる．これまでに関節リウマチ患者14人15手（IP関節7手，MCP関節8手）に対して手術を行い全例に良好な骨癒合が得られているので報告する．

　A technique for metacarpophalangeal (MCP) or interphalangeal (IP) joint arthrodesis of the thumb with an Acutrak^® bone screw in rheumatoid arthritis is described. IP joint arthrodesis is achieved by driving the screw across the joint so that it exits the head of the proximal phalanx, through the base of the distal phalanx, and into the medullary canal, and the joint is fixed in 10° to 20° of flexion. This technique has been used in 14 patients (15 joints) with rheumatoid arthritis and bony union was achieved in every case.
