 The present article documents a case involving a flexor tendon rupture caused by an implant for the internal fixation of a distal radius fracture. The rupture extended from the point of dorsal plate fixation to the distal radius. Six months after surgery, active thumb flexion was impossible. We think that the restrictive thumb flexion was caused by damage to the flexor pollicis longus (FPL) tendon arising from the protrusion of a screw from the palm edge of the distal radius. The plate was removed, and the FPL was reconstructed using a palmaris longs tendon graft. Anatomically, the distal palmar edge of the radius forms a gliding floor for the flexor tendon directly above the periosteum. Therefore, the protrusion of implant screws from the bone cortex must be restricted in this area.
