　頭蓋内進展および頸部リンパ節，脊椎に遠隔転移したolfactory neuroblastoma（ONB）の1例を報告した。症例は35歳男性。鼻閉感，鼻出血，頭痛および嘔吐で発症した。MRIでは右側鼻腔内から蝶形骨洞，右前頭蓋底に進展する腫瘤を認めた。手術にて腫瘍を全摘出したが，術後2カ月のMRIで腫瘍の再増大，頸部リンパ節への遠隔転移および脊髄播種を認めた。Cisplatin，etoposideによる化学療法および放射線治療を行い，原発巣，転移巣は縮小し，患者は独歩退院した。外来で化学療法を継続したが，術後15カ月目に原発巣は再増大し，結局，肺炎にて死亡した。ONBの5年生存率は50％前後と良好だが，本例のごとく頭蓋内進展および遠隔転移を伴う例の予後は不良である。手術と放射線療法による局所療法に加え，cisplatinを中心とする化学療法による積極的な全身性のコントロールを組み合わせて治療すべきではないかと考えている。

Olfactory neuroblastoma is a rare tumor originating in the upper nasal cavity. It rarely extends intracrani-ally. We report a clinical case of olfactory neuroblas-toma with intracranial extension and distant metasta-sis.

A 35-year-old man complained of nasal stuffiness and bleeding, headache and vomiting. Neurological examination showed anosmia and papilledema. MRI showed a huge mass that occupied the right nasal and paranasal cavities, and extended into the right frontal base.
