　症例は71歳の女性。11年間にわたる右顔面痙攣のため来院。MRIでは右顔面神経root exit zone（REZ）を動脈瘤様に拡大した椎骨動脈が圧迫していた。脳血管撮影では右椎骨動脈の後下小脳動脈分岐部末梢に紡錘状動脈瘤を認めた。手術では後下小脳動脈の末梢で椎骨動脈のclippingを行った。Clipping後，脳動脈瘤をREZより移動させると右前下小脳動脈の枝によるREZの圧迫を認めた。スポンジを用いて脳動脈瘤と前下小脳動脈をREZより離し神経減圧を行った。術後顔面痙攣は消失した。椎骨動脈瘤による顔面痙攣は本例を含め5例の報告があるが，脳動脈瘤と右前下小脳動脈の両者の圧迫例は本例が初めてである。5例の特徴について考察した。

We report a rare case of symptomatic hemifacial spasm caused by a fusiform vertebral artery aneurysm and by a branch of the anterior inferior cerebellar ar-tery compressing the facial nerve at the root exit zone (REZ).

A 71-year-old female had an 11-year history of right hemifacial spasm. MRIs demonstrated an aneu-rysm compressing the facial nerve at the REZ. Angio-graphy disclosed a fusiform aneurysm of the right ver-tebral artery at the origin of the posterior inferior cerebellar artery.
