　解離性脳動脈瘤の報告が散見されその病態および自然経過が解明されつつあるが，そのほとんどが頭蓋内それも椎骨脳底動脈系に発生するとされており，頭蓋外椎骨動脈解離性動脈瘤は少ない。症例は44歳男性で，突然の後頭部痛および左片麻痺およびしびれ感にて発症したが，翌日には症状が消失しTIAを呈した。脳血管撮影では，V3 segmentに解離性動脈瘤と思われるいわゆるpearl and string signおよびV2 seg-mentに不整狭窄像を認めたが，発症4週間後には瘤の消失および狭窄像の改善を確認した。頭蓋外椎骨動脈解離性動脈瘤の診断，病因，治療につき若干の文献的考察を加えて報告する。

Spontaneous dissecting aneurysm of the extra-cranial vertebral artery is not frequent. We report a case of dissecting aneurysm of the left extracranial vertebral artery causing TIA. A 44-year-old man was admitted to our hospital with severe occipital-gia of sudden onset on Dec. 21, 1995. On admission, he presented with left hemiparesis and left hemidysesthesia. CT scan and SPECT showed no abnormal findings.
