Ⅰ.目的
くも膜下出血の約80%は破裂脳動脈瘤を原因とするが,その中で解離性動脈瘤は稀である.また,脳動脈瘤の多くはanterior circulationに発生し,椎骨脳底動脈系に発生する頻度は5〜15%と低い.解離性動脈瘤の中でも椎骨脳底動脈系に発生するものに関しては,最近その報告症例数が増加しつつあるが,両側性の症例は少ない.最近われわれはくも膜下出血にて発症した両側性椎骨動脈解離性動脈瘤に対し,手術で両側に対して再出血予防の処置を施したにもかかわらず,慢性期に初回の出血源と反対側の動脈瘤の拡大ならび破裂により死亡するという治療に難渋した一例を経験した.本稿では文献的考察も含め,こうした治療抵抗性の疾患について,さらなる外科的治療の可能性を探ってみたい.
We report a case of a bilateral vertebral dissecting aneurysm associated with subarachnoid hemorrhage. Proximal ligation of the vertebral artery on the ruptured side combined with wrapping of the contralateral dissection failed to prevent fatal rebleeding. Since enlargement of the contralateral dissection was observed by postoperative angiography, rupture of the growing contralateral dissecting aneurysm may have caused rebleeding. Hemodynamic changes following the occlusion of one vertebral artery might have led to enlargement and subsequent rupture of the contralateral dissection.
