Abstract 文献概要

　頸部頸動脈血栓内膜摘除術を施行した43症例でのMatas testを，stump pressure，cross-circula—tionの良否および脳血流（CBF）で評価した。Matas test中のCBFマップは，両側低下群，患側高度低下群，患側軽度低下群，無変化群に分類され，さらにMatas test施行不能群を加えた5つの頻度は各々8，10，14，7，4例であった。患側中大脳動脈領域の血流は，高度低下群で平均37から24ml／100g／minへと最も著明に低下した。施行不能群および高度低下群のstump pressureでは各々平均24,30mmHgと他の3群との間で有意な差が認められた。Cross-fillingがpoorな症例の血流は37から27ml／100g／min，goodな症例では39から34ml／100g／minに低下し，stump pressureと総頸動脈圧の比はpoorな症例が0.32，goodな症例が0.47で有意な差が認められた。以上より，約30％の症例で頸動脈遮断により著明な脳虚血が生じ，criticallevelとして灌流圧では体動脈圧の30％，CBFでは30ml／100g／minが示唆された。

Temporary and /or permanent occlusion of caro-tid artery is one of the most important strategy in the surgical treatment for cervical vascular and paraclinoid lesions. Cerebral ischemia produced by this procedure should be preoperatively grasped for the safety of the surgeries. Matas test has been widely applied to estimate the collateral blood flow at the main artery occlusion by observing only clinical features. In this study, we measured CBF and common carotid artery stump pressure (CCA stump P) in contrast to angiographic cross-filling in 43 carotid endarterectomy (CEA) patients to quan-titatively evaluate Matas test.
