天幕上に発症早期より髄膜播種を呈した脳幹部神経膠腫の1例

Brainstem Glioma with Supratentorial Meningeal Dissemination : A Case Report

　天幕上に早期より髄膜播種を伴ない臨床および画像にて診断困難であった小児脳幹部神経膠腫を経験したので，若干の文献的考察を加え報告した。症例は7歳の女児。主訴は頭痛，発熱，複視，で入院時神経学的異常所見は左外転神経不全麻痺，髄膜刺激症状以外認めなかった。頭部MRIにて脳幹部に占拠性病変を認めたが，Gd-DTPAでの増強は一部に軽度みられたのみで，テント上のくも膜下腔に著明な増強効果を認めた。病初期より髄膜刺激症状とけいれん，精神症状などテント上の症状が前景にたち錐体路症状などの脳幹部の神経症状に乏しかったことから炎症性疾患が疑われ診断に苦慮した。後頭下開頭による脳幹部病変の生検を行いその結果，astrocytoma GIIIと診断された。脳幹部神経膠腫の天幕上播種は極めて稀で，そのMRI像の報告はなく貴重な症例と考えられた。

We present a very rare case of 7 year-old-girlwho had a pontine glioma with supratentorialmeningeal involvement. She complained severe headache with meningeal irritation. She showed fluctuating cranial nerve impairment of the both abducens and glosopharyngeal nerves but no signs of weakness or facial paresis. She also reported two episodes of generalized convulsion with unconcious-ness during admission. MRI disclosed a hypointen-sity intrinsic brainstem mass with an enhancing exophytic component in the prepontine cistern and a sharp contrast uptake is diclosed in the lept-menin-ges of the supratentrial lstructures. An open biopsy was performed and diagnosed as a high grade astrocytoma.
