悪性グリオーマの細胞世代時間（Cell Cycle Time）に関する臨床面よりの解析

ANALYSIS OF TUMOR GROWTH IN RECCURRENT MALIGNANT GLIOMAS

I．はじめに

　脳腫瘍の生長速度を具体的に知ることは，患者の予後の推定，治療方針の決定，あるいは治療効果の判定に寄与するところが大きい。この方面の研究は，Howard andPelc^7）による生長解析の原則が提唱されて以来，正常組織のみならず，種々の実験腫瘍，さらにはヒト悪性腫瘍について行われた。その成果は悪性腫瘍に対する化学療法の改善をもたらし，ある種の白血病の治療効果を飛躍的に進歩させている。

　一般的に，solid tumorの生長解析には種々の困難がつきまとい，特に脳腫瘍の場合には，通常用いられる生長解析の方法が利用できないため，脳腫瘍の生長解析はかなりの遅れをとった。しかし断片的ではあるが，ここ10年間に脳腫瘍の生長解析に関する情報が少しずつもたらされてきている。その多くはH³-thymidine又はC¹⁴-thymidineを用いてradioautographyの手法で解析を行うものであった。初期においては，その解析理論に不充分な点も多かったが，一応の成果としてglioblastoma群で，細胞世代時間（cell cycle time）を45〜60日と推定している^3,8）。しかし近年の報告では細胞世代時間はこれらの値よりも短かくKury and Carter^9）は3〜5日，Tym¹⁰）は5〜10日，Hoshino and Wilson^5,6）は2〜3日という値を算出している。これらの理論上導き出された値の信頼性を検討する必要があり，特に臨床面のデータによる裏づけが重要となってくる。この点に関して，悪性グリオーマ再発例の臨床データを解析することにより，細胞世代時間を逆算し理論値と比較した。

In seven cases of recurrent malignant gliomas treated by extensive removal, the rate of cell growth was analysed in terms of their cell cycle time. On the assumption that the residual cell population is 108 at the time of operation and the growth fraction is 0.3, the range of cell cycle time wascalculated to be 2-3 days. In some cases treated with irradiation and/or chemotherapy after extensive removal, a longer cell cycle time of 3-18 days was obtained.

If we had taken the concept of cell loss factor into consideration, we may have obtained smaller values of cell cycle time in every case. Though these calculated data have many uncertain factors as mentioned above, the cell cycle time of glio-blastoma measured in this way corresponds fairly well to the cell cycle time of 2-3 days in glio-blastoma obtained by Hoshino and Wilson.
