高血圧性脳出血の血管撮影上にみられる静脈出現の異常

VENOUS FILLING ABNORMALITIES IN HYPER-TENSIVE INTRACEREBRAL HEMORRHAGE

はじめに

　脳卒中の急性期の脳血管撮影で—部局所の静脈が他の部より早期に出現する現象を観ることがある^{2）〜6）8）9）12）14）16）17）18）}この所見の観察結果を脳出血と脳硬塞の対比検討としてまとめ，私共は先に報告した^19）。この現象は一過性であることが一つの特徴として認められている。従ってこの現象は脳卒中発症を起点として，発現から消失に至る経過を辿る訳で，個々の症例に当つて読影判定する際には，検査時期との噛合いによつては正常パターンとの差が明確なものから正常範囲のバリエーションとの区別が微妙になるものまでが存在することになる。私共のこれまでの経験では^19），特に脳出血についての早期出現の静脈を指摘判定の明確さは脳硬塞に比べて乏しい印象を受けていた。また，この所見の従来の報告は脳硬塞を素材とするものが大部分で，脳出血を系統的に取扱つて検討したものはみられないようである。そこで今回は高血圧性脳出血だけを対象とし，静脈出現の異常を個々の症例について検討するのではなく，一群として取扱う方法で^4）7）2〜3の検討を行つたので報告したい。

There are many descriptions concerning early-filling of regional veins in cerebral infarction, but a knowledge of the phenomenon is still limited in hypertensive intracerebral hemorrhage. In order to evaluate the occurrence of venous filling ab-normalities in basal ganglionic hemorrhage at the acute stage and their changes in follow-up studies, a comparative study was made between controls and the hemorrhage-group. The following results have been obtained. In the first angiographies carried out at the mean interval of 4 days after onset of hemorrhage, abnormally early appearance was frequently observed in the veins around the hematoma. The phenomenon had no significant correlation with the prolonged circulation time of the brain after the onset. The tendency towa-rds abnormal early filling of the veins involved was on decrease in the follow-up studies perfor-med at the mean interval of about one month after onset in the both groups of operation and on-op-neration, although the patterns were not yet co-mpletely normal.
