外傷性後頭蓋窩血腫

ON THE TRAUMATIC HEMATOMA OF THE POSTERIOR FOSSA

　われわれの経験した，外傷性後頭蓋窩血腫3例を報告，文献上みられた例とともに，後頭蓋窩硬膜外血瞳102例，後頭蓋窩硬膜下血腫25例，小脳内血腫13例をもとにして，病理，臨床症状，診断，予後などについて論じた，これらの血腫は，テント上の血腫と比較して，いくつかの相違点を持つている、すなわち，

　1）　出血源としては，静脈系が多く，亜急性ないし．慢性例が少なくない。

　2）　硬膜外，硬膜下，小脳内の各血腫は，ほぼ同数の，急性、慢例の比率を持つており，テント上の血腫においては，硬膜外血腫のほとんどの例が，急性例であり，また，硬膜下血腫は，慢性例に多いことと，対比されよう。

　3）　一般に診断が困難で，テント上血腫における，超音波診断，あるいは，頸動脈撮影のごとき，決め手となる方法がなく，診断が遅れ勝ちである、われわれは，1例ではあるが，椎骨動脈撮影を行ない，診断に資するところ大であつたが，今後，おおいに行なわるべき，診断法と考える。

　4）　手術成績は意外に良く，最近激増している頭部外傷の中には，後頭蓋窩血腫がそれほど，まれでなく存在すると考えられ，テント上血腫とともに，後頭蓋窩血腫も常に考慮に入れて，診断，治療にのぞむべきと考える。

Traumatic hematomLta of the posterior fossa are considered to be rare, but it is not always the fact. Recently reports of these hematomata are increasing and we should always consider the possibility of these hematomata in dealing with head injuries.

We reported here cases of traumatic extradural, subdural and intracerebellar hematomata of the pos-terior fossa, one case each. Including our own cases, we collected from the literature 102 cases of extra-dural, 25 cases of subdural and 13 cases of intra-cerebellar hematomata of the posterior fossa and discussed their statistics, pathogenesis, clinical fea-tures, treatments and prognosis. We pointed out several differencies between supratentorial and infra-tentorial hematomata.

Among diagnostic aids, the lateral view of the vertebral angiogram may be very reliable.
