自発性低血糖発作ならびにコルサコフ症状群を呈した下垂体顫毛上皮嚢腫の1例

A case of pituitary ciliated epithelial cyst with spontaneous hypoglycemic attacks and Korsakow's syndrome

　1）自発性低血糖発作をくりかえした症例を剖機することによつて，トルコ鞍内および鞍上部にまで発育した約揖指頭大の下垂体顛毛上皮嚢腫を発見した。下垂体はこの嚢腫により下方に圧迫され，萎縮に陥つていたが，視床下部はその前端部が軽度に圧迫されているのみで，組織学的には特にいちじるしい変化は認められなかつた。

　2）下垂体頭毛上皮嚢腫の文献的考察から，本症例のように巨大に発育し，臨床症状を呈することはきわめてまれであることが判明した。特に下乖体頭毛上皮嚢腫によつて自発性低血糖症状を呈したという報告はこれまでにみあたらない。

　3）本症例の示した自発性低血糖発作は下垂体前葉機能不全のためと考えられるが，発作問歌時に認められたコルサコフ症状群と思われる特異な精神症状の発生機転について若干の考察を行なつた。

A case of pituitary cyst lined by a single layer of ciliated columnar epithelium with marked psychic symptoms is reported. The symptoms was characterized by failure of vision, anorexia, repeated hypoglycemic coma, mentally by disturbance of memory, disorientation, phantastic confabulation (namely, Korsakow's syndrome) and inclining to drowsiness.

The cyst was a dumbbell-shaped tumor with both intrasellar and suprasellar parts, the constriction being at the diaphragma sellae and compressed the optic chiasma, the hypophysis and slightly the hypothalamus anteriorly. Microscopically, remarkable lesions were not found in the brain, especially in the hypothalamus.

It is very rare that a cyst of hypophyseal origin attains sufficient size to produce clinical symptoms as our case.
