下垂体上皮性嚢腫の1例

Pituitary Cyst-Case Report

Ⅰ．はじめに

　下垂体嚢腫はGoldzicher^10）（1913）の1剖検例報告にはじまり，Frazier^9）（1934）の手術成功例も報じられ，以来その臨床的あるいは剖検的報告例も増加したが，なお稀なものである．即ち本邦報告例は最近まで皆無でありたが，昨年とさらに本年は本誌6巻5号にYoshidaら^19），小林ら^20）は本邦第1例としての本症1例の外科治療を報じ，われわれの症例は本邦第2例に当たるものであろう．

　さていわゆる鞍傍嚢腫性疾患の大部分は下垂体腺腫に合併する変性嚢腫で，一部頭蓋咽頭腫もみられるが，何れもそれぞれの腫瘍として診断，処置され特に問題はないが，本症即ち下垂体上皮嚢腫は稀であり，発生異常的なものか，腫瘍か否かなど，臨床的，病理学的問題をもつ，最近われわれも本症の1例を経験し，頭蓋咽頭腫の疑いで外科的治療を行なったので，報告すると共に考察をこころみる．

Cyst found in or near the hypophyseal fossa have been believed to occur most commonly as the result of degeneration within pituitary adenomas or craniopharyngiomas partially and are now considered to present little problem. On the other hand, intrasellar cysts lined by a single layer of epithelium developing large enough to give rise to clinical signs and symptoms have been reported in lite-ratures very infrequently. In the present paper we would like to present a patient with a pituitary cyst lined by a single layer of epithelium ciliated in places and was operated on sucessfully.
