認知療法的かかわりにより腹部症状の緩解維持効果が得られた，うつ病を併発した潰瘍性大腸炎の1症例

Cognitive Therapy Improved the Symptoms of Ulcerative Colitis and Major Depressive Disorder

抄録

　潰瘍性大腸炎は大腸粘膜の炎症性疾患であるが，粘膜障害の重症度と臨床症状の重症度との間にかい離がみられる場合があり，心理社会的ストレスへの曝露や人格の偏りが臨床症状の重症度や再燃に関与すると考えられている。

　我々は，潰瘍性大腸炎の緩解と再燃を繰り返した後に大うつ病性障害を合併し，認知療法を試みたところ，潰瘍性大腸炎の消化器症状の緩解期間の延長が得られた症例を経験した。粘膜病変には変化がないにもかかわらず，それまで再燃を繰り返していた潰瘍性大腸炎が緩解状態を維持していたことから，認知療法的かかわりが潰瘍性大腸炎の消化器症状に対し軽減効果を示したと考えられる。

　潰瘍性大腸炎は，併存する精神疾患に対してだけではなく，その身体症状に対しても精神科医が多分に治療的関与するべき疾患であると考えられたので報告する。

　Ulcerative colitis is an inflammatory disease of the large intestinal mucosa. The extent of mucosal inflammation is not always concordant with the clinical severity of the symptoms. It is thought that an exposure to psychosocial stressor and deflection of the character such as anxiety, obsession, irritation affects the clinical symptoms of the disease.

　We report the case of a patient with ulcerative colitis whose symptoms were remitted by cognitive therapy, and the good elapse continued. This case developed major depressive disorder after remission and exacerbation of the ulcerative colitis. Despite mucosal exacerbation, the clinical symptoms of the disease improved. This case suggests that cognitive therapy is effective in reducing the clinical symptoms of ulcerative colitis.

　Therefore, psychiatrists should play an active role in the treatment of ulcerative colitis.
