抄録

　統合失調症治療の幻聴をはじめとする精神病症状に対する心理社会的治療は，今後いっそう充実させなければならない領域の一つである。本症例報告では，幻聴体験から生じる苦痛や不適応を，認知の修正や自己を支持することなどによって軽減しようとする認知療法的介入が行われた慢性統合失調症の2症例を提示した。患者は高齢女性でごく長い期間の幻聴が持続しており，介入の効果が比較的確認しやすい症例だと考えられた。介入は9か月間の約20回の個人面接の中で実施された。そこでは，活動範囲の拡大などの臨床的改善や対処法の発展といった介入の効果と考えられる変化が認められた。我々は今後，幻聴に対する認知療法的介入の改良や，その応用可能性についての議論を進め，その効果を実証していくことが必要である。

　Cognitive therapy for psychotic symptoms is one of the promising areas in psychosocial treatment for patients with schizophrenia. In this report, the authors present two cases of patients with chronic schizophrenia who underwent an individual cognitive intervention for their persistent auditory hallucinations. The intervention was carried out in 21-22 sessions during a 9-month period. The main objectives of the intervention were (1) to make the hallucinatory experience less distressing and frustrating by modifying the underlying pathological cognition, and (2) to provide support for “self” functions of the patients. Some improvements in social activities and enhancement of the patients' coping with the experience were recognized during the treatment, which were supposed to be gains from the intervention. Further efforts are needed to advance treatment techniques, and to examine the efficacy of the intervention for patients with schizophrenia.
