　症例は49歳，女性．40歳頃眼前暗黒感を自覚したことがあり，検診にて単発の心室性期外収縮を指摘されたが経過観察となっていた．1999年2月検診にて高血圧を指摘され近医を受診した際，ふらつき感と多形性心室頻拍を認め当院入院となった．器質的疾患は認めなかったが，入院後も右室流出路起源と思われる単形性の期外収縮で始まる非持続性多形性心室頻拍を頻回に認めた．薬効評価では有効薬を決定し得なかったため，多形性心室頻拍の根治目的に期外収縮を対象としてカテーテルアブレーションを施行した．ペースマップおよび先行度を指標として右室流出路中隔側にて通電したところ期外収縮は消失し多形性心室頻拍も認めなくなったため，無投薬で退院となった．右室流出路起源の期外収縮から多形性心室頻拍への移行例に対しカテーテルアブレーションが有効であった症例を経験した．

A 49-year-old female was admitted to our hospitalfor evaluation of polymorphic ventricular tachycardia(PMVT). The PMVT was always triggered by aventricular premature beat (VPB) which originatedfrom the right ventricular outflow tract. The VT couldnot be induced by programmed ventricular stimulation.Antiarrhythmic drugs (class Ia, Ib, IV and nicorandil)were ineffective for suppression of VPBs, but PMVTwas completely eliminated by catheter ablation target-ing for the triggering VPB. During a follow-up period ofthree months, VT did not recur even though antiarrhyth-mic drugs were not used. Catheter ablation is very usefulfor the treatment of polymorphic ventricular tachycar-dia initiated by a ventricular premature beat originatingfrom the right ventricular outflow tract.
