二腔心に合併した左心症の1例

A Case of Cor Biloculare Associated with Levocardia.

I．緒言

　単心房単心室よりなる二腔心は，魚類にみられる心臓の形式であって，哺乳類では胎生期の心発生第二期において，中隔が全く形成されない場合に生ずる高度の心畸形である。一方，内臓転位を伴う左心症も稀な畸形であり，他の重篤な心畸形を合併していることが多いといわれており，いずれも内外文献に散見するにすぎない。私共は，最近，11才の左心症の症例に，かなり正確な術前診断を下し，Blalock氏短絡手術を施行したが，術後3日目に至り心不全のため不幸な転機をとった1例を経験し，剖検の結果，全く中隔を欠如せる二腔心であることが判明したので報告すると共に，治療方針について文献的考察を加える。

Blalock's shunt operation was done on a 11 year old boy of levocardia associated with situs inversus of the abdominal viscera.

After 3 postoperative days patient died of acute cardiac insufficiency and autopsy re-vealed that there was a true cor biloculare with a single A-V orifice and a right pul-monary stricture which was compensated by a patent ductus arteriosus.

ECG findings were observed in connection with theories of intraventricular conduction and a choice of surgical procedure was discussed.
