Abstract 文献概要

要旨　慢性心不全の薬物治療はACE阻害薬，利尿薬，β遮断薬の併用が一般的であり，さらにアルダクトンやジギタリスの効果も報告されている．しかし，腎不全を合併した心不全症例では，ACE阻害薬による腎障害の増悪，カリウム貯留性のアルダクトンは高カリウム血症，腎排泄低下によるジギタリス中毒などの危険があり，心不全の基本となる薬物治療ができない場合があり，したがってその治療方針も確立していない．今回，腎不全合併のためACE阻害薬が使用できず，心不全により入退院を繰り返す症例でpimobendanとcarvedilolの併用が奏効した1例を経験した．腎不全を合併した心不全例にpimobendanとcarvedilolの併用が有効であったとする報告は本報告が第1例目と考えられ，今後腎不全を合併した心不全に対する治療方法を考えるうえで意義のある症例と考え報告する．

Summary

　ACE antagonist, diuretic and β blocker with the occasional addition of digitalis and aldactone are the commonly used agents for the treatment of chronic heart failure. However, in the case of chronic heart failure complicated with chronic renal failure, we are often unable to use ACE antagonist, digitalis and aldactone because of their side effects. Because of this, it is difficult to know how to manage such patients. This is the first report that the use of pimobendan and car-vedilol together was useful for maintainig patients who have repeated admissions to hospital due to heart failure complicated with chronic renal failure. We think that this report is a meaningful contribution to the solution of the problem of how to treat patients with chronic heart failure complicated by chronic renal failure.
