要旨●実臨床で遭遇頻度の高い上皮性腫瘍と鋸歯状病変に関しては，内視鏡診断と病理組織像の検討は十分に行われ，その診断学は確立していると言える．一方で，大腸非腫瘍性ポリープの疾患頻度は低いが，より多彩な疾患が含まれている．ほとんどが癌化せず，人の生命を侵害する危険性もほぼないという点からも診断体系が確立しているとは言い難い．本稿では大腸非腫瘍性ポリープ6病変について，過去の報告を参考にしながら，臨床的・内視鏡的特徴とその鑑別について概説した．多彩な所見や類似した所見を呈し，確定診断は病理診断に委ねざるをえないものも多いため，内視鏡観察による鑑別の有効性のみならず限界点も認識すべきであると考えられた．

　The endoscopic diagnosis and histopathology of epithelial tumors and serrated lesions, frequently encountered in clinical practice, have been well studied, and their diagnostics are well established. However, benign colon polyps are less frequent and include wide variety of diseases. Most of them are noncancerous and pose little risk to human life ; therefore, it is difficult to say that their diagnostic system is well established. We herein review the clinical and endoscopic features and differentiation of six benign colon polyps, with reference to previous reports. As most of the lesions are present with diverse or similar findings, and the definitive diagnosis must be left to pathological diagnosis, the validity and limitations of endoscopic differentiation should be recognized.
