要旨●大腸の非上皮性腫瘍性ポリープの内視鏡診断のポイントは以下の通りである．発生部位と病変の単発/多発を考慮する．病変の大きさと立ち上がり，粘膜面の評価，EUS所見などから，病変の局在を推定する．生検鉗子などを用いて，硬さを評価する．特徴的な色調から疾患を推察する．表面の性状，境界明瞭/境界不明瞭，陥凹/潰瘍/びらんの有無を観察し，腫瘍露出部があれば拡大内視鏡所見により評価する．さらに大腸粘膜下腫瘍様病変の表面の“不整のない拡張血管”の観察は，内視鏡診断に寄与する可能性がある．

　The key points for the endoscopic diagnosis of colorectal nonepithelial neoplastic polyps are as follows：consider the site and number of lesions, estimate the localization of the lesion based on its size and border, evaluate the mucosal surface and assess endoscopic ultrasonography findings, evaluate the hardness of the lesion using a bioptome, draw inferences regarding the current disease based on the characteristic coloration, observe the surface texture, clear/opaque borders, and the presence of depressions/ulcers/erosions, and evaluate any exposed tumor tissue using magnifying colonoscopy. Furthermore, the observation of “dilated vessels with no irregularities” in the superficial layer of submucosal tumor-like lesions in the colorectum may assist in reaching an endoscopic diagnosis.
