要旨●大腸癌は粘膜内で上皮細胞から発生，浸潤した後に粘膜下層に浸潤すると考えられる．これが正しければ，大腸粘膜内癌は粘膜内浸潤癌と非浸潤癌に分けられるはずである．しかし，国際的に大腸粘膜内浸潤癌の病理学的理解は希薄であり，粘膜内に浸潤する腫瘍に対してpTisすなわちcarcinoma in situに分類するのが現状である．しかし，carcinoma in situという言葉を浸潤性病変に用いることは病理総論的に不適切である．そこで現状を改善するため，粘膜内浸潤癌の存在認識を共有し，粘膜内浸潤癌の診断における問題点を共有したい．また，最近のゲノム解析の情報から，大腸癌の多様性に関する報告について概論しつつ，早期大腸癌の病理診断への取り込みを模索したい．

　Colorectal cancer cells originate from epithelial cells within the lamina propria. Subsequently, cancer cells invade into the lamina propria, resulting in submucosal invasion. Therefore, intramucosal carcinoma restricted to the lamina propria can be divided into invasive and non-invasive types. However, there is little information regarding colorectal cancer restricted to the lamina propria, and all these lesions have been classified as pTis even in cases of invasion. Here, we present information regarding intramucosal carcinoma invading the lamina propria to share our knowledge and experience through a multi-institutional study.
