炎症性ポリープ―内視鏡診断の立場から

Endoscopic Diagnosis of Colorectal Inflammatory Polyps

要旨　広義の炎症性ポリープには炎症性疾患を背景に形成されたポリープと炎症細胞浸潤や肉芽組織によって構成されるポリープが含まれる．本稿では，炎症性腸疾患に伴う炎症性ポリープ，肉芽性ポリープ，化膿性肉芽腫，若年性ポリープ，inflammatory myoglandular polyp，inflammatory cloacogenic polyp，inflammatory fibroid polypにつき，それぞれの概念，病理組織学的特徴，内視鏡所見について述べた．特に若年性ポリープ，inflammatory myoglandular polypは過去の報告では混同されている可能性があり，資料の見直しが必要と考えられる．

　The term, inflammatory polyps, are classified various types of lesions, including those developed on the back ground of inflammatory bowel diseases and those made of inflammatory components. In this article, the concepts, histological characteristic and endoscopic features of the following types of inflammatory polyps are described as inflammatory polyp（or polyposis）accompanying inflammatory bowel diseases, granulation polyp, juvenile polyp, inflammatory cloacogenic polyp and inflammatory fibroid polyp. Among them, there is a possibility that inflammatory myoglandular polyps are mistaken for juvenile polyps. Reappraisals of the clinicopathological features of these polyps are necessary.
