過誤腫性大腸ポリープ―病理診断の立場から

Histopathological Examination of the Hamartomatous Colonic Polyp

要旨　大腸の過誤腫性ポリープである若年性ポリープとPeutz-Jeghers型ポリープについて，臨床病理学的に検討した．ポリポーシス症例は除き，若年性ポリープ56症例60病変，Peutz-Jeghers型ポリープは4症例4病変を対象とした．臨床病理学的な特徴は，いずれのポリープともこれまでの報告と同様であった．病理組織学的には，Peutz-Jeghers型ポリープに比べ，若年性ポリープはinflammatory myoglandular polyp，炎症性ポリープ，mucosal prolapse syndromeなどの鑑別すべき病変が多く，組織学的な特徴を的確にとらえて診断することが必要である．

　There is Peutz-Jeghers type polyp and juvenile polyps in hamartomatous polyps of the colon. Those polyps were investigated clinicopathologically in this paper. Juvenile polyps of 56 patients 60 lesions and Peutz-Jeghers type polyp of four cases 4 lesions were examined. Polyposis cases were excluded, Polyps were studied morphologically, and the gastric and intestinal mucin phenotype was examined. The clinicopathological features of both polyps were similar to the previous reports. Histopathological diagnosis of Peutz-Jeghers type polyp is relatively easy, whereas juvenile polyp needs differentiation with inflammatory myoglandular polyp, inflammatory polyp and mucosal prolapse syndrome. So the point of histolopethlogical diagnosis of juvenile polyp is described in this paper.
