術後狭窄性食道炎

　術後狭窄性食道炎(P. S. E.)は稀ではあるが食道以外の手術に際してみられる重大な合併症である．世界の文献上²⁾39例を数える．われわれの経験では22症例で，内訳は胆摘4，遠位胃切除4，前立腺摘出2，結腸半切除3，経横隔膜迷切2，腸切除2，乳房切断1，子宮切除1，外反母趾手術1，穿孔性胃潰瘍手術1，急性膵炎手術1例であった．1例では迷走神経切断術が経横隔膜的に行なわれたが，他はいずれも胃食道接合部は直接手術侵襲を受けていない．

　Table 1はP. S. E.の診断を確定させる^4)～7)X線検査および内視鏡検査の対象となる症状を示している．Table 2にはわれわれの22例でみられた考慮される原因をまとめてある．注意深く既往歴を調べると，多くの症例では長期間に術前からあったこく軽症の食道への逆流症状が認められた．われわれの22例中4例では以前に受けたX線検査写真がみつかり，食道裂ロヘルニアがあった．14例は鼻腔から胃内への術後吸引¹⁾²⁾が認められた．

　Twenty two cases of P. S. E. are presented and analyzed the probable etiological factors. Most of them had a stormy postoperative course with nasogastric aspiration. Symptoms are outstanding when the patients starts bucal feeding. Dilatation with cooutchouc sounds were used and no patient had to be operated. The precautions to avoid this serious and rare complication of extraesophageal surgery are revised.
