Ⅱa型のX線診断

X-ray Diagnosis of Ⅱa Type Early Gastric Cancer

はじめに

　早期胃癌のX線検査理論は白壁ら^1)～12)によって確立され，Ⅱa型を発見するには透視下圧迫が最良の策であると指摘された．そして，Ⅱa型の大きさ別の発見法が示された．あとは，さがしだすという意欲と，透視診断力および圧迫撮影の技術を身につけることが検者自身の問題となってくる．隆起の大きさが10mm，高さが1mmあれば透視下にみえるという目標も示されているのである．

　さて，Ⅱa型隆起がひろいあげられたとき，つぎのような所見を検討することになる．正面からみた形と輪かくの性状，隆起の高さの程度，表面の性状，隆起の硬さなどである．

　隆起型で深達度smのものは，リンパ腺転移あるいは肝転移が多く，予後は悪い^14-17)．Ⅱaのmかsmかの深達度の診断¹²⁾が重要なわけである．また，組織学的にはⅡaと境界領域の病変^18)～22)のとり扱いが問題になっている．現在のところ，いちおう癌に準ずる変化として扱われているが，両者が肉眼的に鑑別できるものなら，X線で鑑別してみたいとおもう．とにかく，Ⅱa型隆起を発見したら，その肉眼所見を手術前に忠実にあらわしておきたいというわけである．

　もちろん，Ⅱa型隆起とひと口にいっても，その外観様相にはちがいがある．これをいかにX線写真に描写していくかについては工夫を要するところである．検者によっては，工夫のニュアンスがいろいろだろうと考えられる．本文では，各論的に，2，3工夫を要するところを述べてみたいとおもう．

　Theoretically speaking, x-ray examination method of early gastric cancer is now brought to completion. Its differential diagnosis is almost perfected as well. X-ray examination of the stomach is now carried out on the basis of such an accomplished method. In practice, however, several expedients must be still triecl in the correct evaluation of surface aspects of Ⅱa variety of early cancer.

　The items that should be closely investigated are the shape and contour of cancer lesion seen en face view, local difference in unevenness of protrusion, its characteristics visualized en face and its degree of rigidity. The best procedures for these purposes are compression method which delineates any finding as small as respective area gastricae, and double contrast method.

　The degree of rigidity of protrusion is best assessed by its relationship with phases of peristalsis, the relation between the degree of compression and formation of radiolucency, and its relationship with the behavior of mucosal folds in the adjoining areas.
