Patient: male, 58 years of age.

Main complaint: feeling of dull pressure in the epigastrium.

This is a case with two plateau-like protuberances in the atrophic mucosa, both of which were accompanied with atypical epithelium on the surface. The one has a size of 12×9×3mm located on the anterior wall of the incisura, and the other has a size of 9×7×1mm on the anterior wall of the pyrolic antrum.

In this article the process of diagnosis prior to the operation is analyzed.

X-ray diagnosis: The one in the gastric angle was found under fluoroscopy, and its outward appearance was visualized by compression method in both upright and prone positions. The double contrast technique was also employed in prone position.

The other prominence in the antrum was not detected by fluoroscopy, but was revealed by double contrast method (prone). Subsequent compression study enabled us to ascertain the extent and the aspect of the lesion.

GTF diagnosis: The first one was easily detected and observed, while the second one was quite elusive, and twice-repeated examinations aiming at the region where it was supposed to be failed to confirm its existence.
