陥凹型早期大腸癌の微細表面構造―拡大電子スコープ，実体顕微鏡の腺口形態の解析より

Minute Surface Structure of the Depressed Type Early Colorectal Cancer

要旨　陥凹型早期癌は隆起がないことから発見が難しい病変である．しかし本病変は小さくとも深達度進展の速いものが多く，臨床的に最も注意すべきものである．陥凹型病変の実体顕微鏡と拡大電子スコープのpit pattemの特徴について述べた．拡大電子スコープは病変のpit patternを観察するものであり，それにより限りなく組織診断に近い内視鏡診断が可能になった．このことは内視鏡診断学の一層の飛躍を示すものであると考えられた．

　It is difficult to detect the depressed type of colorectal cancer early because of the lack of elevation of the lesions. However, early cancers which are depressed are clinically very important because they extend rapidly to underlying layers even though the lesion itself may be small. We analyzed the characteristic findings of depressed colorectal cancer known as the pit pattern with magnification endoscopy, comparing it with stereomicoroscopic examination. The magnification endoscopy enabled us to detect pit patterned lesions compatible with the stereomicroswpic diagnosis. These resuits suggest that the magnification endoscopy will improve the accuracy of endoscopic diagnosis.
