要旨　新しい内視鏡的治療法の1つの分野である内視鏡的乳頭括約筋切開術は，1973年に開発され既に12年を数えるに至った．ここで本手技開発の経過・海外の動きを紹介するとともに，本法の評価に関して残された幾つかの間題を指摘するとともに今後の研究の進め方について2，3の気付くことを述べた．

　Endoscopic sphincterotomy (EST) was devised in 1973 as one of the therapeutic approach of the endoscopic treatment, and has been put into practice over these 12 years. The author explained the course of the development of EST and the movement in this field in overseas countries, moreover pointed out some problems about the evaluation of EST that have not yet been discussed.
