表層拡大型を呈した胃悪性リンパ腫の1例

Malignant Lymphoma of the Stomach Showing of Superficial Spreading Form

　胃悪性リンパ腫の肉眼形態について，佐野¹らは胃癌との鑑別診断に重点をおいて，表層型，潰瘍型，隆起型，決潰型，巨大皺襞型の5型に分類している．われわれは表層拡大型の1例を経験したので，そのX線，内視鏡，切除標本の肉眼所見および組織学的所見について報告する．

　症　例

　患　者：38歳，女．

　主　訴：むねやけ．

　家族歴：母が子宮癌で死亡．

　既往歴：特になし．

　現病歴：1972年10月28日，国立がんセンター受診．患者はその約1年半前に，空腹時のむねやけを主訴として某医を訪れ，胃X線検査，胃カメラなどの検査を受けた．特に異常は指摘されず，薬物の投与を受けて，一時的に自覚症状は改善した．しかし，1カ月前から同様のむねやけが再び出現した．むねやけは食後2～3時間で始まり，飲食物の摂取により軽快した．また，ときには心窩部痛もあり，胃内に“かたまり”を感じるようになったため患者は胃癌を心配し来院した．当院で胃X線検査，胃カメラおよび生検により，胃悪性リンパ腫と診断され，手術をすすめられ入院した．

　The patient, a 38 year-old japanese female, had occasionally complained of heartburn at the fasting time for about one year and was referred to our hospital for detailed examination in November 1972.

　X-ray and endoscopic examinations of her stomach were performed and she was diagnosed as malignant lymphoma. Also, histological diagnosis with biopsy disclosed as the same.

　Then, a partial gastrectomy was performed. Histological examination of the specimen revealed a reticulum cell sarcoma with the shallow invasion as deep as to the submucosa.

　She has been after this treatment, in good health, then the survival became over eight years.
