胃平滑筋腫瘍の治療法の選択―腹腔鏡下手術

Laparoscopic Treatment of Myogenic Tumors of the Stomach

要旨　胃平滑筋腫瘍に対する内視鏡的治療，特に腹腔鏡下手術について述べた．腹腔鏡下の胃平滑筋腫瘍摘出術としては，胃の漿膜側からアプローチする方法と粘膜側からアプローチする方法（腹腔鏡下胃内手術）がある．漿膜側からの胃平滑筋腫瘍摘出術には，漿膜を切開して腫瘍を核出する方法と胃壁全層を部分切除する方法とがある．前者は粘膜を損傷しないように摘出する手技であり繊細なテクニックを要するが，全層切除と異なり切除後の管腔の狭窄を来す可能性が低い．後者は胃壁を全層にわたって切除できるため腫瘍の完全摘出が容易にできるという長所がある反面，噴門近傍および幽門輪近傍は切除後の狭窄を来す可能性があり，適応を慎重に決定する必要がある．胃内からの腹腔鏡下胃粘膜下腫瘍摘出術は，腹腔鏡下胃内手術のテクニックを応用して胃内腔から腫瘤を切除するもので，胃平滑筋腫瘍でも胃内腔に向かって発育したものが適応となる．

　Laparoscopic management of myogenic tumors of the stomach was described. Myogenic tumors of the stomach can be approached from the serosal or the mucosal side, depending on the case. Extirpation of myogenic tumor of the stomach from the serosal side includes enucleation of the tumor by incising the serosa, and partial resection of the stomach by resecting the whole layer of the stomach. The former method requires skillful technique to avoid injury to the mucosa, however, it has a lower possibility of causing stricture of the gastric lumen. The latter method can remove the tumor completely by resecting the entire layer of the gastric wall, but it may cause stricture of the gastric lumen when applied on a lesion located near the cardia or pyloric ring. Laparoscopic intragastric surgical technique can be indicated for tumors growing into the gastric lumen.
