胃癌の新肉眼分類における使用上の留意点―臨床診断の立場から

Notes on Usage of the New Macroscopic Classification of Gastric Cancer: From the Clinical Diagnostic Viewpoints

要旨　胃癌の新肉眼分類における使用上の留意点について，臨床診断の立場から症例を中心に検討した．この新分類により，次のような点が改善されたと考えられた．①いわゆる早期癌類似進行癌の分類・表記の問題が解決された．②Borrmann分類からの脱却により混乱が避けられる．③TNM分類を併用することで，より国際的に通用するものになった．④臨床・手術・総合所見という各段階での記載により，本来の病変が明瞭となり，また統計処理の向上も期待できる．また，この分類を実際に使用する際には，X線および内視鏡による正確な病変の描出と診断能の向上が必要である．特に隆起型癌の深達度診断の確立と，胃壁収縮が生じていない初期のスキルス型癌（latent linitis plastica型癌）の臨床病理学的特徴を生かせる肉眼分類とその表記法が今後の課題と考えられた．

　We analyzed cases of gastric cancer for the purpose of evaluating the new macroscopic classification of gastric cancer from the clinical diagnostic points of view. This new classification would improve the following points: 1) The issue of classification and notation of so-called advanced cancer mimicking early cancer was solved, 2) We could free ourselves from Borrmann's classification and avoid its confusion, 3) The new classification would be more easily accepted internationally because of combined use of TNM classification, 4) Records of clinical, surgical, and conclusive findings by the new classification would disclose true form of lesions and would improve statistical analysis. Precise description of lesions and improvement in the diagnostic ability of radiologic and endoscopic examinations may be necessary for applying the new classification. Especially evaluation of depth of invasion of protruding lesions needs to be established, and macroscopic classification and notation which represent clinico-pathological characteristics of early scirrhus type cancer (latent linitis plastica cancer) without gastric wall contraction need to be solved in the future.
