要旨　十二指腸の内視鏡治療は10～20mmのポリープ，癌，腺腫，カルチノイド，脂肪腫などに行われている．同部位は壁が薄く，管腔も狭いため治療が困難である．方法は2チャンネルの内視鏡を用いる方法と，1チャンネルの内視鏡を用いる方法が行われている．前者は把持鉗子にて牽引摘除する方法であり，後者は内視鏡先端にキャップを装着して吸引摘除する方法である．手技として，ポリペクトミー，内視鏡的粘膜切除術，ligating deviceを装着して切除する方法，留置型スネアを病変の基部に装着して切除する方法などの報告がある．偶発症を防止するには病変の下部に生食水などの液体を十分に注入して病変を持ち上げる必要がある．

　Duodenal lesions (polyp, cancer, adenoma, carcinoid, and lipoma) ten to twenty mm in size were reported to be able to be resected endoscopically. The therapy is more difficult in the duodenum than in the other portions of the digestive tract because of the thin wall and the narrow lumen. Endoscopes with either one channel or with two channels are used for resection. The treatment is done by means of methods such as polypectomy, endoscopic mucosal resection, the method using a ligating device, and the method with a detention type snare. Perforation and hemorrhage are important complications encountered in endoscopic resection of duodenal tumors. To avoid perforation, sufficient volume of solution should be injected under the lesions.
