要旨　上部消化管出血に際して実施される内視鏡検査について，原因疾患，内視鏡検査の適応・禁忌，検査の手順などについて述べた．緊急内視鏡検査は，通常の内視鏡検査に比べて患者の全身状態が不安定であることが多い．その点で，より慎重な内視鏡操作や観察が望まれる．出血に対しては種々の方法を選択して止血を行うが，内視鏡的止血術に固執し患者の生命を脅かすことがあってはならない．外科的手術への移行など，その他の手段を選択するタイミングを的確に判断することも極めて大切である．

　We spoke of causative disease, the suitability of or contraindication for endoscopy, and the investigative procedures carried out in endoscopy for bleeding in the upper gastrointestinal tract. Compared to normal endoscopy, emergency endoscopy tends to find the patient's condition in an unstable state. However, it is desirable that careful endoscopic investigation and observation should be carried out. When we find upper gastrointestinal hemorrhage on emergency endoscopy, we use various methods to stop the intestinal bleeding, but if we persist in endoscopic hemostasis only we might endanger the patients life. The timing of a change to other hemostatic methods e.g. surgery is of atmost importance.
