Helicobacter pylori除菌後のUl-IV胃潰瘍の経過―3年以上の症例を中心に

Clinical Course of Ul-IV Gastric Ulcer after the Eradication of Helicobacter pylori: Special Attention to Cases which were Followed-up More Than Three Years after Eradication

要旨　これまでHelicobacter pyloriiの除菌を159例に行い，除菌成功後1年以上が経過した胃潰瘍は59例であった．本検討での対象は超音波内視鏡検査によりUl-IVの潰瘍と確認され1年以上経過観察された胃潰瘍18例で，3年以上の観察が8例(最長観察期間4年2か月)であった．除菌後3年以上経過すると，S₂期の瘢痕として内視鏡的に観察された．また，超音波内視鏡検査により潰瘍底部での潰瘍エコーの推移をみると縮小はみられるが，その縮小の程度はわずかであった．除菌により潰瘍の再発が抑えられるため，内視鏡的には潰瘍のステージはより強固な治癒になると考えられるが，潰瘍内部では緩徐な治癒が進行していることが示唆された．

　We aimed to clarify the influence of Helicobacter pylori (Hp) eradication on gastric ulcers by endoscopy and endoscopic ultrasonography (EUS) . Eighteen cases of gastric ulcers which were followed-up more than one year after the eradication. Out of them, eight cases were followed-up more than three years after eradication. These ulcers were diagnosed as penetrating the proper muscular layer EUS. Endoscopically, gastric ulcers were observed as the S₂ stage of ulcer scar more than three years after eradication. On the other hand ulcer echo of these cases was diminished slightly. A discrepancy between the gastric cure at the surface and the cure at the ulcer base was observed.
