short segment Barrett's esophagusから発生し，異形成を伴う早期Barrett食道癌の1例

Early Barrett's Cancer with Dysplasia Occuring in Short Segment Barrett's Esophagus, Report of a Case

要旨　short segment Barrett's esophagusから発生した深達度sm1，高分化型腺癌の1例を経験した．最長2.5cmのほぼ全周性のBarrett上皮内に17×14mmの隆起型（0-Ⅰ）の早期Barrett食道癌でBarrett上皮の一部にdysplasiaを伴っていた．p53の免疫染色を行ったが，癌腫，dysplasia，Barrett上皮のいずれにも染色性を示さなかった．short Barrettにも発癌のpotentialがあり，注意を要すると考えられた．

　We encountered an early Barrett's cancer case with dysplasia occurring in short segment Barrett's esophagus. 0-Ⅰ type of well differentiated adenocarcinoma was recognized in Barrett's epithelium of which the longest part was 2.5 cm from the esophago-gastric junction. The depth of invasion of the tumor was limited within the shallow layer of the submucosa. Moreover, dysplasia was also recognized in the same Barrett's epithelium. p53 immunostaining was performed, but Barrett's cancer, dysplasia and the other parts of Barrett's epithelium did not stain.

　Short segment Barrett's esophagus is said to havemalignant potential, so careful observation and follow-up is recommended.
