要旨　現在の電子スコープ中，最も画素の多いEVG-HRのルーチン検査としての使用経験から，早期胃癌診断への応用についての検討を行った．画像は他器種より優れているが，早期胃癌の性状診断では分化型胃癌の範囲の診断，Ⅱbの診断には問題が残っている．微小胃癌の診断には今なお有用性は示されていない．しかし，今後の電子スコープの進歩はこれらの問題を解決していくものと思われる．

　Diagnosis of early gastric cancer is described with reference to our experience in using EVG-HR. Present problems involved and prospects for the development of the electronic endoscope are discussed.

　The image of EVG-HR using the present electronic endoscope was excellent for the upper gastrointestinal tract.

　However, detection of Ⅱb, and the diagnosis of the border of some differentiated type Ⅱc was difficult.

　There have been no minute gastric cancers in our experiences of a sequence of 3,000 examinations so far, and we are afraid that these may be overlooked because of the phenomenon of high light imaging by the electronic endoscope.

　To counteract thus, we often use a dye scattering method in electronic endoscopy for the diagnosis of early gastric cancer, and we are also trying the use of a magnifying electronic endoscope.

　In the future, a more efficient electronic endoscope will be developed as electronic technology progresses. With this improved electronic endoscope, we expect that minute gastric cancers will be detected more easily through upper gastrointestinal endoscopy.
