要旨　若年者の上部消化管病変は，中高年者とは異なった病態を示すことが少なくない．さらに若年者の中でも，いわゆる15歳未満の小児と20歳代の若年成人との間には，やはり大きな差がある．小児期では胃癌の頻度は極めて低いこと，H. pylori感染の頻度は10％弱であり多くは結節性胃炎を呈することなどの違いがある一方で，NSAIDsやストレスによる胃・十二指腸病変は同様に存在する．小児期における内視鏡検査の難しさが，正確な病態把握を困難にしているものと思われるが，小児期から成人に至るまでの消化管免疫機構の変化を十分に把握することが，両者間に存在する相違点の解明に必要と考えられる．

　Upper gastrointestinal lesions in young persons have different pathophysiology compared to those of older patients. In young persons, there are also some differences in the lesions between children and young adults. In children, the incidence of gastric cancer is very low and H. pylori infection is detected in less than 10％ of the population and usually causes nodular gastritis. However stress and NSAIDs induced gastroduodenal injury are equally detected in children. Difficulty of endoscopic examinations is one of the important reasons why insufficient evaluations are made of gastroduodenal lesions in children. Further evaluation of immunological changes in gastrointestinal mucosa from pediatric to adult periods should be performed.
