要旨　患者は48歳，男性．検診の胃X線検査で異常所見を指摘され，当科を紹介された．胃X線，内視鏡所見では，胃体下部大彎に立ち上がりのなだらかな隆起性病変を認めた．表面は平滑であり，頂部は発赤し，その一部に浅いびらんを呈していた．クッションサインは陽性で，軟らかい粘膜下腫瘍と考えられた．超音波内視鏡所見では，病変は第3層に主座を置く，粘膜下腫瘍として描出された．内部は単房性の無エコー域であり，また第2層から粘膜筋板を越えて第3層に存在する嚢胞に突出したやや低エコーな領域を認めた．治療として内視鏡的粘膜下層剥離術を実施した．病理組織学的に，単房性の胃hamartomatous inverted polypと診断された．

　A 48-year-old man was admitted to our hospital for further examination of a polypoid lesion of the stomach. X-ray and endoscopic examinations revealed a submucosal tumor at the greater curvature of the lower body of the stomach. Endoscopic ultrasonography demonstrated anechoic legion in the third layer of the gastric wall and low-echo nodule in the lesion. ESD（endoscopic submucosal dissection）was performed to make an accurate diagnosis and select optimal treatment. ESD achieved complete resection of the tumor. Histologically, the polypoid lesion consisted of gastric hamartomatous inverted polyp.
