要旨　早期胃癌の肉眼型での複合型は基本型が複合した病変に対して用いられる．胃癌取扱い規約ではより広い病変から順に「＋」記号でつないで記載することが決められている．しかし，肉眼型にはそれが用いられるようになった経緯があり，解釈に差がみられることがある．特に複合型の0 IIa＋IIc型，0 IIc＋IIa型について顕著である．すなわち，0 IIa＋IIc型では0IIa部分が0 IIc部分を環状に取り巻いているという形態的な特徴を重視する考えがある．また，0 IIc＋IIa型では病変の一部に潰瘍を伴う0 IIc型の癌があり，ほかの部，特に0 IIcの周辺に0 IIa型がみられる肉眼型を呈する例に対して用いるという考えがある．

　Lesions of early gastric cancers that revealed combined type consisted of two fundamental types or more. According to the Japanese classification of gastric cancer, the type occupying the larger area should be described first, followed by the next type. But these macroscopic types are interpreted slightly differently between institutions. Especially the difference of interpretation is wide in lesions of type 0 IIa＋IIc or type 0 IIc＋IIa, because there is an opinion that the macroscopic characteristics are important. In this opinion, type 0 IIa＋IIc should be used for lesions in which the portion of 0 IIa surrounds circularly the portion of 0 IIc macroscopically. Lesions of type 0 IIc＋IIa are those in which the portion of 0 IIc with ulcer is accompanied with the portion of 0 IIa.
