要旨　従来，通常観察とヨード染色による食道内視鏡検査で早期食道癌の拾い上げ診断が行われてきた．近年，拡大内視鏡の普及により食道疾患の質的診断にも応用されている．食道疾患を拡大観察することにより良悪性の鑑別も可能になってきた．ヨード染色で初めて発見される平坦な不染部，逆流性食道炎に伴う食道胃接合部領域の隆起性変化，Barrett食道における粘膜面の変化の診断などが対象となる．治療が難しいとされる食道癌も内視鏡的治療の進歩により大きく変化している．ゆえに，食道疾患の良悪性を鑑別しての早期診断は臨床上，極めて重要である．

　Conventional observation and iodine staining have been employed for detection of early esophageal cancer. Magnifying endoscopy has recently been used for diagnosis of esophageal lesions. Usage of the magnifying endoscope enables also the distinguishing of malignant lesions from benign lesions. The flat iodine unstained lesion, the protrusion on the esophago-gastric junction accompanied by reflux esophagitis, the mucosal change on Barrett's epithelium are among the lesions frequently under discussion.

　Development of endoscopic technique is changing the treatment of esophageal cancer. Detection of esophageal cancer in its early stage is very important.
