早期食道癌存在診断・質的診断の進歩―通常内視鏡による診断：高画質内視鏡

Development in Diagnosis of Localization and Character of Early Esophageal Cancer─Diagnosis by Conventional Investigation Employing High Resolution Endoscopy

要旨　ヨード染色を中心とした色素内視鏡に加えて，現在では特殊内視鏡，さらに拡大観察が開発され，診断技術が著しく進歩している．しかし，現時点においても高画質通常内視鏡観察による病巣診断が最重要視されるべきであると考えている．食道癌の深達度診断は通常内視鏡検査を基本とした病変の深さ，高さ，広さ，色調，辺縁の立ち上がり，上皮下発育の有無など総合的に判定されるべきであると考えている．本稿では，高画質内視鏡の開発とHi-Vision電子内視鏡システムの特徴，次に早期食道癌に対する高画質内視鏡を用いた通常内視鏡観察による存在診断と質的診断について，その有用性を述べた．

　Besides chromoendoscopy including iodine staining, endoscopic techniques have been making tremendous progress. Among them, we would like to recommend high resolution endoscopy enalling more accurate diagnosis of depth of invasion, protrusion size, subtle change of color, wedge of the tumor, and subepithelial growth of the tumor. This chapter summarizes the introduction of high resolution endoscopy such as the Hi-Vision endoscopic system and its characteristics, as well as its usefulness in conventional investigation of the localization and character of the tumor.
