Endoscopic Mucosal Resection for Early Esophageal Cancer

要旨　早期食道癌に対する第一選択の治療法としてEEMR-tube法による内視鏡的粘膜切除術（EMR）を施行してきた．一括切除が可能な病巣では，極めて短い治療時間で容易に手技を完了することができる．大きな病巣に対しては，安全で確実な分割切除法としてEEMR-tube 4段法を行っている．第1段階では病巣の口側より最深部が含まれるように切除する．第2段階では残存する範囲を同様の手技で，繰り返しの追加切除を行う．第3段階は辺縁部の小範囲切除でEEMR-tubeのサイドチャンネルを利用しダブルチャンネルの要領で切除する．最後の第4段階では，ホットバイオプシーやAPCによるトリミング処置を施行し治療を完了する．

　We are performing endoscopic mucosal resection (EMR) employing the EEMR-tube method as the treatment of choice for early esophageal cancer. This maneuver can be completed easily in a very short time when the whole lesion is removed by one action. A large lesion is managed by the EEMR-tube 4 step method, requiring several actions to avoid leaving any unresected area. The first step is to remove the lesion containing the deepest focus from the oral side. The second step is to repeat this removal process on the residual area. The third step is to remove the subtle surrounding area by a double channel method using a side channel. The fourth step is to complete the removal by trimming and employing hot biopsies and APC.
