転移性胃腫瘍の形態的特徴―X線像を中心に

Radiographic Findings of Metastasis to the Stomach

要旨　胃転移性腫瘍の形態的特徴についてX線像を呈示して述べた．胃の転移性腫瘍の多くは脈管を経由する血行性転移で，原発巣としては肺癌，食道癌，乳癌などあり，隣接臓器からの直接胃壁への浸潤はまれである．血行性転移の形態は基本的に粘膜下腫瘍の形態であり，頂部が潰瘍化して中心陥凹を形成しやすい．腫瘍径に比較して大きくて深いのが特徴でbull's eye sign（牛眼像），target sign と称される．また，腫瘍がまだ小さい例でもすでに明らかな潰瘍形成を来すたこいぼびらん様の小隆起として観察され，病変が多発すれば診断が容易である．乳癌の転移では胃壁が硬化し，粘膜ひだが肥厚するlinitis plastica様所見を呈することもある．転移性胃腫瘍は比較的小さい時期には無症状なため，臨床で胃転移が診断されることは少なく，ほとんどは剖検で発見されてきたが，今後は検査においてこれらの所見を十分認識する必要があると考えられた．

　Metastatic carcinoma of the stomach was investigated and it's radiographic findings were discussed. Morphologically, hematogenic metastasis from the lung, esophagus or breast is often observed but direct invasion from the neighboring organs is very rare. Most of the hematogenic metastases manifest themselves as submucosal tumors with big and deep ulcerations at the center of the tumor. It's characteristic figure on barium meal study is usually called “bull's eye sign” or “target sign”. Even when metastatic tumors are small, they still appear as an elevation with central depression like a gastric erosion. They are hard to diagnosis because they haven't any typical symptoms, but we need to look for this typical clinical findings during ordinary examination.
